Confirm the place
Match the name on this website with the signboard and the area. Call the number on the listing if you are unsure. Maps links, when shown, use the address stored on the listing.
- Name of the clinic, laboratory, or pharmacy
- Area and street address
- Phone number answered by that provider
- Booking window or hours, confirmed by phone if you are travelling far
What to carry
Carry a photo identity card if the provider asks for one. Carry previous prescriptions and laboratory reports that belong to the same person. Carry a written list of medicines you already take, including the dose printed on the strip. Do not rely on this website to store that list unless you have a Health Card and you choose to keep notes there.
- The patient’s name and a mobile number the clinic can call
- Earlier prescriptions and reports, if you have them
- Any referral slip from another doctor
- A payment method the provider accepts — ask in advance
If someone is seriously ill
Do not use search, a token, or a contact form for a medical emergency. Dial 108 for ambulance help. Many states also publicise 102. If the call does not connect, go to the nearest hospital emergency department.
This checklist does not tell you whether a symptom is an emergency. That judgement belongs to the emergency service and the clinicians who see the patient.
